There are 6 different application forms:

1. Application for a temporary residence permit

2. Application to change the conditions of a temporary residence permit

3. Application to extend the period of validity of a temporary residence permit

4. Application for a legal declaration

5. Change of address form

6. Application for a not applicable declaration

There are different types of permit request:

A1 Work or Stay

A2 Change or Renewal of A!

A3 After 5 years

A4 Indefinte

B Dutch citizens

C Dutch students

D Dutch citizens sent by Government

E Foreigners sent by Government

F Foreigners born in the Netherlands Antilles

G For non applicable requests

H For Family reunion

I1 For Marriage with a Dutch citizen

I2 Renewal of I1

J1 For Application as a director

J2 Renewal of J1

K1 For Study

K2 Renewal of K1

L Application of penshonado status

M Short Stay (6 months)

Per application, besides the requirement list, all applications need to be accompanied by one or more declarations.

These are the different decalarations (coloured balls):

1. Declaration of Employment

2. Guarantee

3. Declaration of Relationship

4. Declaration of Marital status

5. Study Declaration
